Use this page to submit an RMA Form to Mattec's Repair Department. Submit one form for each MIU that will be sent in for repair.

You will not be notified of the cost of a repair in advance unless the cost exceeds the Standard Repair Charges. The Standard Repair Charges cover most items, but do not cover circuit boards, LCDs or keypads. If a repair is not covered by your maintenance contract, please include a Purchase Order Number below.
You are not required to wait on a RMA number before sending in your unit for repair. As soon as you submit this form you may send the unit to Mattec. Standard procedure is not to generate a RMA number until the repairs are received at Mattec.
Please submit this form elctronically using the button on the bottom of the page. Do not print this form and include it with the unit. This form will be emailed to the repair department and an useable RMA form is generated automatically.
